Microsoft Exchange Server Elevation of Privilege and NTLM Relay Vulnerability (CVE-2018-8581) Analysis & Mitigation Guide
Executive & SecOps Summary
SECOPS ASSESSMENTMicrosoft Exchange Server 2010, 2013, 2016, and 2019
An attacker can elevate privileges to SYSTEM/Exchange Windows Permissions, leading to a complete compromise of the Active Directory domain controller
Remove the 'DisableLoopbackCheck' registry key, apply the latest Exchange Cumulative Updates, and enable Extended Protection for Authentication (EPA) on EWS
Affected Products & Version Scope
Remediation & Live Workaround Commands
PATCH & WORKAROUND1. Official Patch & Version Upgrade Commands
Apply the latest Cumulative Updates (CU) and security updates provided by Microsoft. Additionally, ensure that the loopback check registry key is removed to prevent local NTLM relaying.
# Remove the DisableLoopbackCheck registry key to mitigate local relaying
Remove-ItemProperty -Path 'HKLM:\SYSTEM\CurrentControlSet\Control\Lsa' -Name 'DisableLoopbackCheck' -ErrorAction SilentlyContinue
# Restart IIS services to apply changes
iisreset /noforce2. Temporary Workaround & Hardening Configuration
If immediate patching is not feasible, enable Extended Protection for Authentication (EPA) on the EWS virtual directory to block NTLM relay attacks.
# Import WebAdministration module and configure Extended Protection on EWS
Import-Module WebAdministration
Set-WebConfigurationProperty -Filter 'system.webServer/security/authentication/windowsAuthentication' -Name 'extendedProtection.tokenChecking' -Value 'Require' -PSPath 'IIS:\sites\Default Web Site\ews'
# Verify the configuration
Get-WebConfigurationProperty -Filter 'system.webServer/security/authentication/windowsAuthentication' -Name 'extendedProtection.tokenChecking' -PSPath 'IIS:\sites\Default Web Site\ews'3. Log Audit & IoC Verification Commands
Search IIS access logs for suspicious EWS subscription requests pointing to external or non-standard IP addresses.
# Search for EWS Subscribe requests originating from or pointing to external hosts in IIS logs
grep -i 'Subscribe' /var/log/nginx/exchange_access.log | grep -E 'ews/Exchange.asmx' | grep -v '127.0.0.1'Block Kernel Privilege Escalation & Ransomware Payloads Before You Can Reboot
Protect unpatched Windows, Linux, and macOS workloads against in-memory exploitation and lateral movement with autonomous EDR.
Technical Root Cause & Exploitation Mechanics
1. Vulnerability Overview & Root Cause
This vulnerability is an elevation of privilege vulnerability in Microsoft Exchange Server, specifically within the Exchange Web Services (EWS) push notification mechanism. Exchange Server allows clients to subscribe to push notifications, instructing the server to send updates to a specified URL. Due to a lack of proper validation, an attacker can perform a Server-Side Request Forgery (SSRF). Combined with the fact that Exchange Server runs with high privileges (SYSTEM) and possesses extensive permissions ('Exchange Windows Permissions') within Active Directory by default, this behavior creates a severe security flaw.
2. Exploitation Mechanics & Attack Scenario
An attacker with any valid mailbox credentials sends a crafted EWS Subscribe request to the Exchange Server, specifying an attacker-controlled listener as the notification endpoint. The Exchange Server attempts to send a push notification to the attacker's server and authenticates itself using NTLM over HTTP. The attacker intercepts this NTLM credential and relays it (NTLM Relay) back to the same Exchange Server or to an Active Directory Domain Controller (via LDAP/S). Because the Exchange subsystem has high privileges (such as WriteDacl) in the domain, the attacker can modify domain permissions to grant themselves Domain Admin rights.
3. Infrastructure & Business Impact
This vulnerability poses an existential threat to enterprise infrastructure, as it bridges the gap between a compromised low-privilege email account and full Active Directory domain takeover. It is a highly critical flaw actively exploited in the wild by advanced persistent threat (APT) groups, as reflected in its inclusion in the CISA KEV catalog. A successful compromise allows attackers to deploy ransomware domain-wide, exfiltrate sensitive corporate communications, and establish persistent backdoors.
